School Health Services
The Elizabeth School District School Medicaid Reimbursement Program (also known as the School Health Services Program) allows school districts to seek partial reimbursement for health-related services provided to Medicaid-enrolled students who have an Individual Education Plan (IEP) to include other plans of care such as Behavior Plans, Health Care Plans and Health-related 504 plans.

Claiming for health-related services given at school does not impact a families personal insurance benefits, the SHS program is completely separate from individual health insurance policies. Per Colorado’s School Health Services Program, reimbursements to school districts must be used to support health-related services and programs to improve or enhance health and wellness for all students. Services that receive school Medicaid reimbursement are physicians, nursing, personal care, psychology, counseling, social work, speech language, hearing, occupational therapy, physical therapy, and specialized transportation.
The SHS form is a one-time consent that is valid for the duration of your students’ time with the Elizabeth School District. The SHS consent form (English/ Spanish)
