As of Feb. 5, 2024, Elizabeth School District requires CORA requests to be submitted via the form below. This process replaces the fillable PDF which had been used previously. Please note: Previous forms that were downloadable, to be filled out and returned via email or in person, will no longer be accepted. All requests must be submitted via the form below to ensure proper record keeping.
