Over-the-Counter Medication Permission Form
With parent/guardian permission, the school will have the following OTC medications and products available to administer to your child on an as-needed basis: acetaminophen, antibiotic cream, Benadryl, Benadryl cream, burn gel, Claritin, contact solution, cough syrup, ibuprofen, saline eye drops, sunscreen, throat lozenges, Tums, unscented lotion, vaseline, and Zyrtec.

Medication Permission Form
The school nurse & other delegated personnel are required by state law to have this form signed by a parent/guardian and the student's health care provider before any prescription, homeopathic, or OTC medication not listed on the "Over-the-Counter Medication Permission Form" may be given at school.
This form will need to be completed on an annual basis.

IF THE SCHOOL DOES NOT HAVE THE REQUIRED HEALTH FORMS FOR YOUR CHILD'S MEDICAL DIAGNOSIS, WE ARE NOT ABLE TO TREAT YOUR CHILD AT SCHOOL AND WILL NEED TO CALL 911 IF SCHOOL STAFF DEEM IT IS AN EMERGENT SITUATION.

Diabetes Self-Management Agreement
If your student is managing his/her diabetes independently, it is strongly recommended that your student have rapid sugar sources (juice, glucose tabs, cake frosting, candy etc.), extra supplies, and a form of GLUCAGON in the school health office.
