Vaccine Requirements for Colorado Schools & Childcare Facilities

Colorado law (Board of Health rule 6 CCR 1009-2) requires all students attending Colorado schools and licensed childcare facilities to be vaccinated against certain diseases, unless an exemption is filed.
Below are the vaccines required:
Vaccines required to enter Childcare & Preschool:
- Hepatitis B (Hep B)
- Diptheria, tetanus, pertussis (DTaP)
- Haemophilus influenza type b (Hib)
- Inactivated poliovirus (IPV)
- Pneumococcal conjugate (PCV)
- Measles, mumps, rubella (MMR)
- Varicella (chickenpox)
Vaccines required to enter School (K-12):
- Hepatitis B (Hep B)
- Diptheria, tetanus, pertussis (DTaP)*
- Inactivated poliovirus (IPV)*
- Measles, mumps, rubella (MMR)*
- Varicella (chickenpox)*
- Tetanus, diptheria, pertussis (Tdap)**
*Students 4 through 6 years of age should have their final doses of DTaP, IPV, MMR and Varicella prior to kindergarten entry.
**Students should have Tdap prior to entry into 6th grade. One dose of Tdap is required for 6th through 12th grades.

Non-Medical Exemptions (NME)
This is an exemption based upon a religious or personal belief that is opposed to immunizations.
There are 2 ways to obtain a Certificate of Non-Medical Exemption
- Review and complete the Online Immunization Education Module. (This does NOT require the signature of a health care provider).
or
- The Certificate of Non-Medical Exemption can be signed by a health care provider and parent.
NME forms expire each year on June 30th. A new Certificate of Non-Medical Exemption must be submitted every school year.

Medical Exemptions
This is an exemption that is available to students who have a medical condition that prevents them from receiving vaccines.
A Certificate of Medical Exemption requires the signature of one of the following health care providers:
- Advanced Practice Nurse (APN)
- Physician (Medical Doctor [MD] or Doctor of Osteopathic Medicine [DO])
- Physician Assistant (PA)
A Certificate of Medical Exemption needs to be submitted only once.
